Open Enrollment for the UAPB’s health, dental, vision and optional long-term disability, as well as Flexible Spending Accounts begins 11/1/2017 and ends 11/30/2017. This is your once-a-year opportunity to make changes to your benefits.  After November 30, 2017, you can only make changes if you are within 31 days of a qualifying life event. We encourage you to complete any open enrollment changes no later than 11/21/2017 to ensure you will receive any new ID cards by 1/1/2018. ALL MEDICAL AND DENTAL ENROLLEES WILL RECEIVE NEW CARDS EFFECTIVE 1/1/2018.

Health:  Starting January 1, 2018, two new medical plans will replace the Point of Services Plan, and the Classic Plan will continue to be offered as well.  All three plans cover the same medical services and facilities, including no cost preventive care. The plans are Classic, Health Saving and Premier Plan. There is a small increase to Tier 2 and Tier 3 prescription drugs under the Classic Plan.  If you fail to enroll for 2018 and are currently enrolled in the Point of Service or Classic Plan, you will be automatically enrolled in the Classic Plan with the same coverage election you had in 2017. New medical cards will be issued.   Starting January 1, 2018, participants in all three medical plans may save on health care costs through the SmartCare program by visiting a provider at a University of Arkansas for Medical Sciences (UAMS) facility. 

Dental:  Effective January 1, 2018, Arkansas Blue Cross, Blue Shield Dental will become the U of AR System’s dental carrier. The dental coverage will include a new orthodontia benefit for children under the age of 18. The plan will pay 50% up to a lifetime limit of $2,000 per child. Dental premiums will not increase in 2018. New dental cards will be issued.  

Vision Coverage:  Superior Vision premiums will not increase in 2018.   This plan is a discount program for purchasing glasses or contacts.  Select between the Basic Plan and Enhanced Plan.  

Spouse and Dependent Eligibility:  If you are adding your spouse or dependent to your medical, dental or vision coverage, you will need to submit a marriage license for your spouse and birth certificates or legal court documents for child(ren). 

Flexible Spending Accounts:  Enroll in Flexible Spending Accounts for 2018. The maximum amount for Healthcare FSA increased for the 2018 tax year to $2,600.  The maximum for Dependent Care FSA is $5,000.00. With Healthcare FSA you will have until March 31, 2018 to file claims incurred through December 31, 2017.    Remember, annual enrollment is required for FSAs. New Benny cards will be issued for employees with cards that expire 1/2018.  You cannot be enrolled in both Flexible Spending Account and Health Saving Medical Plan.   

Accidental Death & Dismemberment.  Premiums will not increase for the 2018 plan year. Enroll or change your AD&D coverage at any time.  

Basic Life Insurance:    The University of Arkansas provides basic term life insurance for you free of cost. The amount is one times your salary adjusted to the nearest thousand, up to a maximum benefit of $50,000. The employer’s cost will increase for 2018. Benefits are reduced at ages 70 and 75.  

Optional Life Insurance:   There is no open enrollment for optional life insurance. This benefit is paid for by the employee. The premium will increase for 2018. If you would like to enroll or increase optional life insurance, you will need to complete the evidence of insurability paperwork.    

Dependent Life Insurance:   There is no open enrollment for optional dependent life insurance. This benefit is paid for by employee.  The premium will increase for 2018.  Covers you spouse for $10,000, $15,000 or $20,000.  Children from birth to 25th birthday are covered at 50%. If you would like to enroll or increase dependent life insurance, you will need to complete the evidence of insurability paperwork. 

Basic Long Term Disability Insurance:    All eligible employees are automatically enrolled in the University of Arkansas Basic Long Term Disability Plan. The employer pays the premium and there is no cost to you. The premium will increase for 2018.  This plan covers your first $20,000 of salary. The maximum monthly benefit is $1,000.  

 Optional Long Term Disability Insurance:    There is open enrollment for optional Long-Term Disability.  If your salary is more than $20,000, you may enroll in optional LTD without completing the evidence of insurability paperwork. There will be a premium increase for 2018.  

Auto and Homeowners:  Group Auto/Homeowners insurance through Liberty Mutual.  You may enroll at any time. 

Critical Illness Insurance.  There is open enrollment for Critical Illness through MetLife. You can apply for up to $10,000 of coverage for you and your family.  

Identity Theft.  This is a new voluntary product. The University is offering this optional voluntary benefits.  The carrier   is ID Watchdog. This benefit is for protection against identity theft.  See Identity Theft section for more details. 

Prepaid Legal. This is a new voluntary product.  This is an optional voluntary benefit.  The carrier is LegalShield.   This benefit gives you unlimited personal advice from a lawyer on specific topics.  

 

IMPORTANT NOTICE:  IF YOU CHANGE YOUR ADDRESS THRU WEBADVISOR, PLEASE CHANGE YOUR ADDRESS WITH YOUR BENEFIT CARRIERS BY COMING TO HUMAN RESOURCES AND COMPLETING THE NECESSARY PAPERWORK.  THIS WILL ENSURE VITAL DOCUMENTS AND CORRESPONDENCE IS BEING MAILED TO YOUR CORRECT ADDRESS.
